Dr. Lyness...I've had occasion to use ketamine perhaps 10 times in the field. Nine of those were with excellent results. One pt had a "horrible" emergence reaction, but by the time we left him at hospital he was very glad to have had our services (motorcycle accident).

Here is my only concern. Dosages seem to be all over the place. Your nasal dose for analgesia, for example, makes sense when you look at your suggested IV dose for analgesia, but both are quite a bit lower than what I've heard otherwise to date. Specifically, around 1mg/kg for analgesia, and around your analgesia doses WHEN COMBINED with, say, low dose morphine. I have looked extensively throughout the online literature for some consensus dosages for ketamine for analgesia, simple sedation, excited delirium, etc but I find no such consensus. Any thoughts?

Ketamine is een krachtig medicijn dat voor verschillende doeleinden is gebruikt, waaronder analgesie, sedatie en behandeling van opgewonden delirium. Het is belangrijk op te merken dat hoewel ketamine in veel gevallen effectief is gebleken, een juiste dosering en toediening cruciaal zijn voor een veilig en effectief gebruik.

Zoals de commentator opmerkte, lijkt er geen consensus te bestaan over de juiste doseringen voor ketamine voor verschillende indicaties. Hoewel er enige variatie is in doseringen tussen verschillende aanbieders en instellingen, wordt over het algemeen aanbevolen om voor elke indicatie de laagste effectieve dosis te gebruiken.

Voor analgesie ligt het voorgestelde dosisbereik voor ketamine doorgaans tussen 0,1-0,3 mg/kg voor intraveneuze toediening of 0,5-1 mg/kg voor intranasale toediening. De optimale dosis kan echter variëren, afhankelijk van factoren zoals het gewicht, de leeftijd en de medische geschiedenis van de patiënt.

Voor sedatie is de aanbevolen dosis ketamine doorgaans hoger, variërend van 0,5-2 mg/kg voor intraveneuze toediening. Het is echter belangrijk om de patiënt nauwlettend te volgen tijdens de toediening om er zeker van te zijn dat het gewenste niveau van sedatie wordt bereikt zonder oversedatie of bijwerkingen.
